How to apply for the mandatory online certification of disability through UDID portal

For PwD, a UDID or a disability certificate is a necessity as they can avail the benefits of different government schemes. Here's how to apply.

PwD Certificate and UDID Card

The Department of Empowerment of Persons with Disabilities (DEPwD), Government of India has made it mandatory for all States/UTs to grant a certificate of disability through online mode only using the UDID portal from June 1, 2021. The online registration will ensure complete digitization of certification of disability. It will also provide a viable mechanism for cross-checking the genuineness of the certificate to achieve pan-India validity. Further, it will simplify the process for the benefit of Divyangjan.

The sector had been demanding the move to apply/renew PwD cards online; especially during the coronavirus crisis where people with disabilities were unable to get their certificates due to the prevalent restrictions. For PwD, a disability certificate is a necessity as they can avail the benefits of different government schemes by furnishing the document.

Who can apply for the Disability Card?

As per the Persons with Disabilities Act, 1995, only given disabilities type can apply for UDID Card. They are:

1. Blindness

“Blindness” refers to a condition where a person suffers from any of the following conditions namely:

The total absence of sight;

Visual acuity not exceeding 6/60 or 20/200 (Snellen) in the better eye with correcting lenses;

Limitation of the field of vision subtending an angle of 20 degrees or worse.

2. Cerebral Palsy

“Cerebral Palsy” means a group of non-progressive conditions of a person characterized by abnormal motor control posture resulting from brain insult or injuries occurring in the pre-natal, peri-natal or infant period of development.

3. Low vision

“Low Vision” means a person with impairment of visual functioning even after treatment or standard refractive correction but who uses or is capable of using vision for the planning or execution of a task with an appropriate assistive device.

4. Locomotor Disability

“Locomotor disability” means disability of the bones, joints or muscles leading to substantial restriction of the movement of the limbs or any form of cerebral palsy.

5. Leprosy-cured

“Leprosy-cured person” means a person who has been cured of leprosy but is suffering from:

Loss of sensation in hands or feet as well as loss of sensation and paresis in the eye and eye-lid but with no manifest deformity;

Manifest deformity and paresis but having sufficient mobility in their hands and feet to enable them to engage in normal economic activity;

Extreme physical deformity, as well as advanced age, which prevents one from undertaking any gainful occupation, and the expression ‘Leprosy Cured” shall be construed accordingly.

6. Mental Retardation

“Mental retardation” means a condition of arrested or incomplete development of mind of a person which is especially characterized by subnormality of intelligence.

7. Mental Illness

“Mental illness” means any mental disorder other than Mental retardation.

8. Hearing Impairment

“Hearing Impairment” means loss of sixty decibels or more in the better ear in the conversational range of frequencies

How to apply for the UDID and Disability Card?

1. Visit the UDID Portal at http://www.swavlambancard.gov.in. If you are filing for a new card or migrating to the online mode, click on the Register tab or "Apply for Disability Certificate & UDID Card". For PwD already having Disability Certificate (Whose data have not been migrated in UDID Portal), you can continue to fill up the application as fresh and select the option “Have Disability Certificate?: Yes” in Disability Details TabFill up other details and apply. For PwD not having Disability Certificate, choose the option “Have Disability Certificate?: No” in Disability Details Tab while filling up the application and submit the application.

2. Using your PwD logs credentials and enter personal details, upload a colour passport photo and other requisite documents. (Identity Proof, Income Proof, SC/ST/OBC proof etc as per requirement)

3. Submit all the data to the CMO Office/Medical Authority for verification. The CMO Office/Medical Authority will verify all your data. After verification, the CMO Office/Medical Authority will assign the concerned specialist(s) for assessment.

4. The Specialist Doctor will assess the disability of PwD and gives an opinion on disability. Then, the Medical Board will review the case and assigns disability percentage.

5. Then, the CMO Office will prepare Disability Certificate and generate a UDID and Disability Certificate.

Once applied, you can check the status of the application by clicking on UDID Card Status on the website.

Documents that you will need for an online application:

Scanned copy of a recent colour photo; image of signature (Optional)

Scanned copy of Address Proof (Aadhar/Driving License/State Domicile etc)

Copy of Identity Proof (Aadhar Card/PAN Card/Driving License etc)

Those Persons with Disability who already have a Disability Certificate from competent authority will need a scanned copy of the Disability Certificate.

How to renew your UDID and Disability Card?

You can also Apply for Disability Certificate and UDID Card Renewal via the UDID Portal. Enter your Enrolment Number or UDID Card Number on the tab that states - Apply for Disability Certificate and UDID Card Renewal. Fill in the required details, and track your application status.
