To be a part of the nomination process, the questionnaire has to be completed by the participants. A score of 250 is assigned for both parts combined.

Based on the IT Implementation, a simplified questionnaire has been framed.The focus is on one Most Successful implementation. The questionnaire is divided into two parts based on the IT Implementation.

Part 1: Comprises of Introductory Information

Part 2 has 2 Sections of which Section A : focuses on Implementation

Part 2, Section B focuses on the individual category awards.
